TransHis Electronic Patient Record
TransHis is an Electronic Patient Record (EPR) for use in primary care settings. The program contains an extensive help-file, accessible at all times through F1. The program is made using Borlands Delphi®.
It allows comprehensive coding of all elements with the International Classification of Primary Care, second revised edition (ICPC-2R): reasons for encounter, diagnoses and interventions (including medication by ATC code, referrals, and test results) in an episode of care structure. All diagnostic titles can semi-automatically be double coded with both an ICPC-2R and an ICD-10 code. For double coding the diagnosis our ICPC-2/ICD-10 thesaurus can be used.
The medication can be classified using ATC codes or ICPC drug codes. In TransHis, normally Prescriptor® is used, an ICPC-driven drug formulary program developed by Digitalis. In this program, selected drugs are related to the diagnosis (ICPC code), sex and age of the patient, his/her drug intolerances, contra-indications and co-medication. To use TransHis in combination with Prescriptor, you don’t need to install Prescriptor.
Test results can be coded using the latest labcode table of the Dutch College of General Practitioners (NHG, Nederlands Huisartsen Genootschap). Tests are divided in ‘intermediate’ (performed at the practice itself) and ‘resulting’ (ordered for some time after the consultation, to be performed elsewhere) ones. For resulting tests, a pre-selection is offered dependant on the diagnosis.
You can select the language of the program, Dutch and English, by switching in the program itself. There are two databases with demo data. The first database has the name TransHis_NL_Demo and uses the Dutch versions of the ICPC, ICD10, labtests etc. The second has the name TransHis_Uk_Demo and uses the English versions. You can change the database name by editing the initialization file.
A lot of reports are included in the program. The number of reports is increasing rapidly.
